Please Fill Out the Form Completely.  This form is for changing your address with the union only.  Your address change will be sent to District 142 and will be maintained there.  If you need to change your address with the company, please see your base administrator or supervisor.  You may also use the Continental/ExpressJet intranet site to submit changes to the company.  Changes must be made separately for both the company and the Union as each party does not share their respective address lists.
